Community Flu-shot Clinic - Oct 5

In an effort to promote community health and wellness, Chilton ISD has partnered with Walgreens to offer a flu immunization clinic for the community on Monday, Oct. 5 from 11 a.m. to 1 p.m. The clinic will take place at the Chilton ISD Athletic field house located on State Hwy 7 in Chilton. Please follow the link found at www.chiltonisd.org to complete the attached registration form for each individual participating in the clinic.
